What insurances do you accept?

We currently accept the following insurances: Medicare, Medicaid, Aetna, Anthem BCBS, Blue Cross Blue Shield of Michigan PPO, BCBS Medicare Advantage (Medicare Plus Blue), Blue Cross Complete (Medicaid), Blue Care Network/BCN Advantage, United Healthcare/UHC Medicare, Humana and Humana Military, Total Healthcare, Meridian Health/Meridian WellCare (Medicare and Medicaid), McLaren/McLaren Medicaid, MultiPlan, HAP and Priority Health (Medicare, Medicaid, Employer Plans, PPO, HMO, POS).

Do not hesitate to call us to check provider eligibility if you do not see your insurance listed.

What should I bring with me to my first appointment?

Please bring your driver’s license (or a valid photo identification), insurance card, imaging scans if applicable, and patient paperwork (If you choose to complete the forms in office, we kindly ask that you arrive 30 minutes early for your first appointment).

What are your hours of operation?

Monday: 8AM – 4PM
Tuesday: CLOSED
Wednesday: 8AM – 4PM
Thursday: 12PM – 4PM
Friday: 8am- 3pm
Saturday/Sunday: CLOSED
